Richmond Eye Experts and Elite Family Vision are required by HIPAA, applicable Texas law, and other applicable laws to protect the privacy and security of your protected health information and to follow the terms of this Notice.

  • Treatment, including examinations, prescriptions, referrals, coordination of care, laboratories, pharmacies, and other health care providers.

  • Health care operations, including quality improvement, staff training, compliance, audits, credentialing, business management, and medical record administration.

  • We may also share relevant information with family members, caregivers, or others involved in your care or payment for your care when permitted by law.
  • We may provide information to business associates that perform services on our behalf. These organizations are required to safeguard your information.

Uses Requiring Your Authorization

  • We will obtain your written authorization when required by law for uses or disclosures not otherwise permitted by this Notice, including certain marketing activities and any sale of protected health information.
  • You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.

  • Request confidential communications at a specific telephone number, email address, or mailing address.
  • Inspect or obtain a paper or electronic copy of your health information, subject to limited legal exceptions.

  • Request an accounting of certain disclosures of your health information.
  • Request that records be sent to another person or organization when permitted by law.
  • Receive a paper copy of this Notice at any time.
  • Have a legally authorized representative exercise certain rights on your behalf.

Please provide enough information for us to identify you and the records requested. We will respond within the time required by applicable federal and Texas law.

SMS, Phone, and Email Communications

  • We may communicate with you using the phone number and email address you provide on your intake, registration, or other practice forms.
  • Please ensure that the contact information you provide is appropriate for communications from our office. If you provide a spouse’s, parent’s, family member’s, employer’s, work, or shared telephone number or email address, communications from our office may be received by or accessible to that person or organization.
  • If you do not want us to use a particular telephone number or email address, please notify our office.
  • By providing a mobile phone number and opting in to receive text messages from Richmond Eye Experts and/or Elite Family Vision, you consent to receive SMS communications from our practices.
  • Messages may include appointment reminders, scheduling communications, recall notices, office notifications, and other patient-related communications.
  • Message frequency varies. Message and data rates may apply.
  • Reply STOP at any time to opt out of future text messages. Reply HELP for assistance.
  • Your personal information, including your mobile number and SMS consent information, will not be sold, rented, or shared with third parties or affiliates for marketing or promotional purposes.

Complaints and Licensing Information

If you believe your privacy rights have been violated, you may file a complaint with our practice. We will not retaliate against you for filing a complaint.

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights or contact the Texas Attorney General, Consumer Protection Division regarding applicable consumer privacy concerns.

Our Responsibilities

We are required by law to maintain the privacy and security of your protected health information, notify you when required following certain breaches, and follow the privacy practices described in the Notice currently in effect.

Changes to This Notice

We may revise this Notice as permitted by law. Any revised Notice may apply to information we already maintain as well as information we receive in the future.
